All the "Loading ... module" lines I see are:
ext3
ide-core
ide-disk
dock
scsi_mod
libata
ata_piix
ahci
piix
ide_generic

however, I also tried root=/dev/hda5 with the image created by rpm install,
but that did not work either.

Yes, if I rebuild the image either with --preload ide-modules, or --preload
piix, or without any options, every image boots. Just not the one created
by the rpm script: I tried it again just now (removing the kernel and
installing again the same package) and I got the same errors.
